Please Help!
The Office of Protection and Advocacy for Persons with Disabilities (P&A) is looking for input in determining its priorities for the upcoming year. How can you help? Please take a few minutes to complete our 2012 Disability Issues Survey. It is available in multiple formats, including:

Fillable Survey that you can complete on your computer, once completed save a copy to your computer; print it out and either mail or fax to (860-566-8714) P&A; or attach to an e-mail and send it to OPA-Information@ct.gov


 
The fillable survey is also attached to this e-mail in English and also in Spanish. You can fill it out, save a copy to your computer and e-mail it back to OPA at OPA-Information@ct.gov Or you can print out the survey and complete it by hand and fax or mail it to us. Our fax number is 860-566-8714 and the mailing address is at the bottom of this message.

 
Why is your help important to P&A? What you tell us in the survey shapes our work! P&A will use the survey results to assist with determining the issues affecting people with disabilities in Connecticut. The results are combined with P&A experience during the previous year and form the basis for setting the priorities for the agency.
